Social Prescribing
What is social prescribing?
Your well-being is affected by many things, such as housing, income, relationships, and sense of community. Social prescribing looks beyond medical interventions and “prescribes” activities and programs that can support your health and wellness based on your interests, strengths, and what is important to you.
Gateway CHC’s team of doctors, nurse practitioners, nurses, and other health providers works with you to understand what matters to you, then identifies programs and community services that reflect your needs and interests. Social prescriptions help people use their talents productively, develop new skills, and make connections in the community.
What does a social prescription look like?
It could be participating in a senior's group, joining an exercise class, enrolling in a community garden project, receiving a Good Food Box to support food security, participating in community theatre, volunteering at an event or taking an art class.
We work to remove any barriers to participation, whether financial, geographic, or accessibility-related, to support you in participating in activities that are meaningful to you.
