Nurse Practitioners & Family Doctor
Family physicians and nurse practitioners are the first place to turn when you have a non-emergency health concern or simply want to maintain your good health. Gateway CHC has a team of highly skilled Nurse Practitioners and a family doctor to diagnose and treat your health issues, conduct screenings for certain diseases, and arrange blood work or other lab services to assess your health. As a Gateway CHC patient, you get regular access to medical professionals who get to know your personal health and family medical history. This relationship means they can provide you with consistent care based on a deep understanding of your needs and can spot subtle changes that a walk-in clinic might miss.
Our nurse practitioners and family doctor work as part of a coordinated team to help you improve your overall health, prevent future issues, understand your medications, and support you in managing your long-term health conditions.
We offer scheduled medical appointments, as well as some urgent appointments and home visits.
Urgent Care Appointments
We hold a limited number of same-day appointments available for urgent care needs. These appointments are offered on a first-call basis, with calls accepted from 8:30 AM until 11 AM. Your call may be returned by a nurse for further telephone assessment. Urgent care refers to a medical need that cannot wait for a regularly scheduled appointment but is not an emergency. Examples of common reasons for an urgent appointment include:
- A child with a fever and ear pain
- A patient with a fever, coughing, or shortness of breath
- A red painful eye
- An infected wound
If you are experiencing a medical emergency, such as chest pain or difficulty breathing, please call 9-1-1.
General Medical Care
Gateway CHC provides general health care services through a coordinated team of doctors, nurse practitioners, nurses, a pharmacist, social worker, dietitian, and others. They work together to develop a care plan to meet your specific health needs and circumstances. Our primary health care team can help you improve your overall health, prevent future issues, manage your long-term health conditions, and understand your medications. We look at your whole person to consider various things that may be affecting your health and we connect you to other Gateway CHC programs and services that might benefit you. If we don’t offer a specific service that we think would benefit your health, our team can refer you to other organizations and services that do.
