Community Outreach Nurse
The Community Outreach Nurse (Registered Nurse) supports individuals and families in the community by providing care and coordination in the home and in community settings. Through home visits, the nurse helps identify health and social needs, reduce barriers to care, and improve access to services that support overall well-being.
Working closely with individuals, families, and the broader care team, the Community Outreach Nurse helps develop care plans, coordinate services, and ensure smooth transitions between providers. They also connect people to community programs and resources, while advocating for care that is accessible and closer to home.
